what to expect my first visit

                MEDICATION THAT ASSISTS PATIENTS IN TREATMENT 

FOR ADDICTION TO OPIATES AND/OR ALCOHOL. 

   

WE COUPLE THE MEDICATION WITH GROUP AND INDIVIDUAL COUNSELING USING EVIDENCE-BASEDTREATMENt  TECHNIQUES 

                  

 FOR EXTRAORDINARY RESULTS 




 Must have ID and insurance card.  Some type of valid identification is mandatory for the treatment process to begin


We ask that you come with a full bladder, and be prepared for urine drug testing.  We understand that you are coming to Relentless Recovery for treatment and expect illicit substance(s) in the initial drug screen.


We ask that client be in active withdrawal.  Having at least 12 hours since the client's last use.  Withdrawal symptoms must be observed by the treatment team before the medical appointment.  (if methadone, more than 24 hours since dose)


Understand that the initial appointment will be longer than the established appointment, due to the need for the History and Physical form to be completed (you can save time by completing the intake face sheet form below)


Explanation of how initial appointments will be more frequent (multiple telehealth appointments a month initially-every 2 weeks  etc... thereafter.  Depending on the phase.) A clear understanding of regular appointment attendance is required in order to continue participating in the program.


Conversation for clients opting out of the insurance program.  Policy requirements made clear that fees are collected at the time of appointment. CC/Cash/Money order NO CHECKS - NO EXCEPTIONS.  Visits do not include the cost of medication.  


The program is fully explained including pill counts, relapse prevention protocol as well as Relapse protocol


Sign consent forms in order to be completely enrolled in our program.
